Before you attend Tralee Dental and Implant Cliinc for the first time, please complete our medical questionnaire.
What will happen at my first appointment?
When you arrive at the practice, you will be welcomed by one of our friendly receptionists who will ask you for your completed medical history form and smile check list which you will have received in the post. Don’t worry if you have forgotten to bring it with you, we will happily give you another one.
If you have any problems filling in your medical questionnaire, we will give you any help you need. Please bring a list of any medications that you are currently taking. You can then take a seat in our patient lounge and your dentist will come out to introduce themselves shortly. We will then invite you into the treatment room where you will take a seat in comfortable chair and have an informal chat about your personal dental needs. We will take as much time as necessary to give you all the different options available – you choose.
Once we fully understand how you would like your smile to be, we will ask you if we can have a look in your mouth. We may take some X-rays to help us see the full picture. After this we will plan your treatment with you and advise you on the costs and timescales involved. You will then return to see one of our receptionists, who will book the appropriate appointments and give you a written treatment plan and estimate. It’s as easy as that.
